Dialectical Behaviour Therapy

  • Benefits

  • Manage Cravings

  • Regulate Emotions

  • Build Coping Skills

  • Prevent Relapse

Recovery from substance use is rarely just a case of stopping alcohol and drug consumption. You need to change how a person responds to feelings, stress and everyday challenges. One way of doing this is Dialectical Behaviour Therapy, also known as DBT.

At Asana Lodge, we incorporate DBT into individual treatment plans, but suitability is determined through a clinical assessment. This guide reveals what you can expect from the treatment type.

What is Dialectical Behaviour Therapy?

Originally developed by psychologist Marsha Linehan, DBT is closely related to cognitive behavioural approaches. The word ‘dialectical’ refers to the integration of opposing ideas – specifically balancing acceptance and change. In addiction treatment, this means individuals are supported to acknowledge and accept their current thoughts, feelings and experiences without judgement while also learning skills to change harmful behaviour.

Instead of fighting against an internal state, individuals learn to sit with their experiences and look for a constructive way forward. It focuses on practical skill development and gives people the tools they need to apply to high-risk situations both during rehab and after returning home.

How Can DBT Support Addiction Recovery?

There’s a well-known link between emotional experience, impulsive urges and substance use. What’s more, we find that many people with an addiction have used drugs or alcohol as an emotional regulator to try to numb, escape or manage an overwhelming internal state.

DBT for drug addiction and DBT for alcohol addiction help individuals to recognise these cycles. Being able to identify early signs of these triggers in the future means individuals can then practice alternative coping strategies before resorting to substance use.

DBT doesn’t remove cravings or eliminate stress entirely; however, it instead provides a crucial in-between moment – that time between experiencing an urge and acting upon it.

Dialectical Behaviour Therapy for Addiction and Emotional Regulation

DBT can help people to understand and manage intense emotional states that may otherwise lead to a relapse. Through DBT, individuals learn how to:

  • Identify emotions rather than feeling overwhelmed
  • Recognise internal and external triggers
  • Understand how emotions directly influence behaviour and cravings
  • Develop healthier and non-destructive ways to modulate intense feelings

Distress Tolerance

DBT also focuses on helping people to learn how to deal with difficult emotions, physical discomfort and challenging circumstances without resorting to substance use. This might include events, interpersonal conflicts or strong cravings.

The aim isn’t to suppress or deny these emotions but instead to teach people how to tolerate uncomfortable feelings safely until they pass.

Mindfulness in Rehab

Mindfulness is a foundational element of DBT and teaches people to pay attention to the present moment without self-criticism. This includes developing a heightened awareness of:

  • Passing thoughts and internal dialogues
  • Fleeting and intense emotions
  • Physical sensations within the body
  • Cravings and impulsive urges

The aim is to be able to pause between an emotional urge and the physical response. While mindfulness can’t cure or control addiction, it can give people the mental space needed to choose a healthy coping mechanism rather than being reactive.

Interpersonal Effectiveness

Relationships can be hugely helpful during recovery but they can also bring conflict, stress and triggers. This is where interpersonal effectiveness comes in, helping people to navigate social interactions. This might include:

  • How to communicate thoughts and needs clearly
  • Setting and maintaining healthy personal boundaries
  • Managing disagreements and conflict without aggression
  • Asking for help and support when needed
  • Expressing personal needs while respecting the rights of others
  • Maintaining stable and supportive relationships without reverting to old coping behaviours

What Does Dialectical Behaviour Therapy Treatment Look Like at Asana Lodge?

At Asana Lodge, our recovery approach combines one-to-one therapy sessions, weekly keywork sessions, ongoing care reviews and daily therapeutic groups. Where appropriate, this may include Dialectical Behaviour Therapy for addiction.

However, because every recovery journey is different, DBT is delivered as part of a tailored care plan rather than a rigid programme. There are no fixed numbers when it comes to sessions, and instead we adapt the extent and focus of every treatment session for each resident as they pass through the care plan. This may include:

Who Might Benefit from Dialectical Behaviour Therapy for Addiction?

DBT can be a powerful tool, but it’s best for those facing certain emotional and psychological challenges. That’s why our initial admission assessment evaluates whether DBT rehab is appropriate on an individual basis.

Typically, DBT is beneficial if someone experiences:

  • Difficulty managing intense and rapid mood shifts
  • Impulsive behaviours during times of stress or distress
  • Strong and overwhelming reactions to everyday pressures
  • A pattern of repeated substance use following emotions
  • Trouble tolerating cravings
  • Frequent conflict within personal or professional relationships
  • Difficulty pausing to consider consequences before acting

DBT and Relapse Prevention

A key part of your recovery will centre around relapse prevention planning. This is where DBT can be especially helpful. This is because individuals learn how to:

  • Identify specific emotional triggers unique to them
  • Spot early warning signs before reaching a crisis
  • Develop a personalised toolkit of coping strategies
  • Become more aware of high-risk environments and situations
  • Continuously apply mindfulness, distress tolerance and emotional regulation skills in everyday life

It’s important to note that there isn’t a single therapy that can guarantee complete abstinence or eliminate the risk of relapse, but using a combination of practical skills and therapy can empower people to navigate life without turning to drugs or alcohol.
